Epitalon
A guide to the pineal tetrapeptide: forty years of work from one St. Petersburg institute, a telomerase result in a dish, and a July 2026 committee vote on an indication no study has tested
Epitalon (also spelled epithalon or epithalone) is a four-amino-acid peptide, Ala-Glu-Asp-Gly, designed in the 1980s by Vladimir Khavinson's group in Leningrad as a synthetic stand-in for an extract of the pineal gland. Its reputation rests on three claims: that it switches on telomerase in human cells, that it restores melatonin rhythm in the old, and that the pineal preparations lengthened the lives of elderly Russians followed for years. This guide covers what those studies were and who ran them, why almost none has been repeated outside that institute, how the vials are dosed, and what the FDA's advisory committee decided in July 2026 for insomnia, a use FDA's reviewers found no human study of at all.
Who this is for
Written for adults who are using, or being offered, epitalon for sleep, longevity, or 'telomere support', and for the clinics and sellers describing it. It summarizes the published record and the regulatory position as of the edition date. It is not medical advice.
In the 1970s and 1980s, Khavinson and Morozov at the Leningrad military medical academy made extracts of animal organs, the pineal gland and the thymus among them, and gave them to elderly patients as 'bioregulators'. The pineal extract was called epithalamin. Epitalon is the synthetic tetrapeptide they designed to reproduce it, on the theory that short peptides carry the tissue's regulatory signal. The theory is the institute's own, and so is nearly all the evidence.
- Class
- Synthetic tetrapeptide (Ala-Glu-Asp-Gly), a designed stand-in for the pineal extract epithalamin; a 'Khavinson bioregulator'
- Claimed uses
- Longevity, telomere lengthening, melatonin and sleep, immune and endocrine 'normalization'; nominated to FDA for insomnia
- Human evidence
- Small Russian trials and cohorts from one institute, mostly of the extract rather than the tetrapeptide; one independent cell-line replication (2025); no independent human study; no sleep study of any kind
- Forms
- Research-chemical vials of 10 and 50 mg; registered in Russia as a drug; no approved or compounded product in the United States
- Price as sold
- About $30 to $55 for 10 mg and $120 to $150 for a single 50 mg vial (about $50 each in ten-vial kits); a seller course uses 50 to 100 mg
- Status
- Not approved. FDA's advisory committee voted 7 to 4 with an abstention in July 2026 to recommend it for the 503A list, against FDA staff's recommendation; the agency has not acted
- Anti-doping
- Prohibited at all times as a non-approved substance (S0)
One institute
Search the literature and nearly every human result on epitalon or epithalamin carries the same names: Khavinson, Morozov, Anisimov, Korkushko. That is not proof the work is wrong. It is the reason a claim forty years old is still unconfirmed in people; the one outside confirmation, in 2025, is in cultured cells.
Three lines of evidence support the claims, and each has a catch: the cell result was confirmed elsewhere only in 2025, in a dish, with a finding about cancer cells attached; the animal results are from the same group, and the human cohorts were of the extract, not the tetrapeptide, and were not randomized.
| Study | What was done | What it showed | Catch |
|---|---|---|---|
| Khavinson et al., 2003 (cells) | Epitalon added to cultured human fibroblasts | Telomerase activity switched on; telomeres lengthened | Repeated once outside the institute, by a Brunel University group in 2025, in cell lines; that study also found telomere lengthening in breast cancer cells |
| Anisimov et al., 2003 (mice) | Epitalon injections to female mice from middle age | Slower aging markers, fewer spontaneous tumors; no longer lifespan overall | Same institute; modest effect |
| Khavinson and Morozov, 2003 (266 elderly) | Thymalin and epithalamin extracts, courses over 6 to 8 years | Lower mortality in treated groups | The extract, not the peptide; not blinded; not replicated |
| Korkushko et al., 2006 (elderly with 'accelerated aging') | Epithalamin courses over years | Better function and melatonin rhythm; lower mortality | As above |
| Any study of sleep | None | FDA's 2026 review found no human study of epitalon in insomnia by any measure | The nominated use has no evidence |

Telomerase, in a dish and in a person
The telomerase result is a cell-culture finding, repeated in 2025 by a UK group that also saw epitalon lengthen telomeres in breast cancer cell lines. No human study has measured telomere length before and after epitalon. Telomerase is the enzyme most cancers switch on to keep dividing; a drug that activated it in the body, and that lengthens cancer cells' telomeres in a dish, would need cancer safety data that does not exist.
| Form | Instruction | What is known |
|---|---|---|
| Vial, 10 mg | 5 to 10 mg daily, subcutaneous, for 10 to 20 days; repeat once or twice a year | Modeled on the Russian short courses of the extract; no dose-finding in people for the tetrapeptide |
| Vial, 50 mg | As above; one vial is a course | As above |
| Low-dose daily | 1 to 3 mg daily, ongoing | A seller invention |
| Nasal, oral | Various | Absorption unmeasured |
What the next table is, and is not
It expresses the common seller course for a 10 mg vial so the arithmetic can be checked. The Russian human work used the extract by intramuscular injection in short courses; the tetrapeptide's human dose has never been established. A vial's identity, strength, and sterility are unverified.
- Vial
- 10 mg lyophilized epitalon, as sold
- Water added
- 1 mL bacteriostatic water
- Concentration
- 10 mg/mL. One concentration for the whole vial.
- Syringe
- 1 mL U-100 insulin syringe
| Days | Dose | Draw | Units | Used per course |
|---|---|---|---|---|
| 1–10 | 10 mg daily | 1 mL | 100 | 100 mg (10 vials) |
| or 1–20 | 5 mg daily | 0.5 mL | 50 | 100 mg (10 vials) |
| Between courses | Nothing for 4 to 6 months |
A course is 100 mg either way, about $250 to $300 in single 50 mg vials. Sellers describe two courses a year; the Russian cohorts were dosed in courses too, but with a different preparation and no published rationale for the interval.
What each draw looks like · 1 mL (100-unit) U-100 insulin syringe
The plunger tip sits on the line for the dose. Units are marks on a U-100 syringe: 100 units is 1 mL. The picture assumes the concentration in the table above; a different amount of water changes every line.
Epitalon was nominated to the 503A bulks list for insomnia, on the melatonin claim. FDA's briefing document for the July 23 and 24, 2026 meeting reported that its reviewers found no human study of epitalon in insomnia by any measure, that the evidence came from a single Russian laboratory with limited independent replication and much of it in Russian-language journals, and that safety and characterization were uncertain. Staff recommended against listing. The committee voted 7 to 4, with one abstention, to recommend it anyway, alongside BPC-157, TB-500, KPV, MOTS-c, and Semax.
| Question | Answer |
|---|---|
| Is epitalon approved? | No, and the vote is not an approval |
| Can a pharmacy compound it now? | No. The committee advises; FDA decides by rulemaking, which has not happened |
| Is the research vial now legal to use? | No. It remains an unapproved new drug |
| Is it approved anywhere? | Registered in Russia, on the institute's own data |
| What did FDA's own scientists say? | No sleep study exists; one laboratory; substantial safety and characterization uncertainty |
Reading the headline
Sellers report the vote as FDA recognizing epitalon for sleep. The record says the reviewers could find no sleep study, and a panel recommended listing it anyway. If FDA follows the panel, a pharmacy could compound it on a prescription. Nothing about the evidence changes either way.
Reading a vial: milligrams and units8
Whole-milligram doses make the arithmetic easy. The 50 mg vial is the pitfall: reconstituted with 2 mL it is 25 mg/mL, and a 10 mg dose is 40 units rather than a full syringe.
units = (dose mg ÷ concentration mg/mL) × 100
A 10 mg vial with 1 mL is 10 mg/mL: 5 mg is 50 units, 10 mg is 100. A 50 mg vial with 2 mL is 25 mg/mL: 10 mg is 40 units. Write the concentration on the vial.
| Vial and water | Concentration | 5 mg | 10 mg |
|---|---|---|---|
| 10 mg + 1 mL | 10 mg/mL | 50 units | 100 units |
| 50 mg + 2 mL | 25 mg/mL | 20 units | 40 units |
| 50 mg + 5 mL | 10 mg/mL | 50 units | 100 units |
A 50 mg vial opened for a 10-day course is used within the course; refrigerate. Stability of the reconstituted peptide over longer periods is unpublished.
- Reported
- The Russian papers report no significant adverse effects of the extract or the peptide over years of courses; the reports are brief and unblinded. Seller forums describe injection-site soreness, drowsiness, and vivid dreams.
- Telomerase
- If epitalon activated telomerase in the body as it did in the dish, the theoretical risk is to any cell that should not keep dividing. No human cancer safety data exist. The mouse study found fewer tumors, which cuts the other way, but in one strain from one group.
- Hormonal
- Claimed to raise melatonin and alter gonadotropins and cortisol in the elderly; the effects were measured in small groups by the same institute.
- Who should not
- Pregnancy, anyone under 18, and anyone with cancer or a cancer history, on the telomerase question alone.
- Product
- A tetrapeptide is cheap to make and hard to verify without testing; independent assays of gray-market peptides have found mislabeled and underfilled vials across the category.
- Unknown
- Everything, outside one institute's reports: long-term effects, interactions, and whether the tetrapeptide does in people what the extract was reported to do.
- What am I trying to change? If it is sleep, do I know that no sleep study of epitalon exists?
- Do I understand that the human evidence comes from one institute and was never repeated?
- Do I have cancer or a cancer history? Telomerase is the reason to ask.
- What is the concentration on this vial, and my dose in units?
- Where did the vial come from, and is there an independent certificate of analysis for this lot?
- Am I tested for anti-doping? Epitalon is prohibited at all times.
- For sleep: have I tried the things with trials first?
What does have evidence for what epitalon is sold for
For insomnia: cognitive behavioral therapy for insomnia, with decades of trials, then the approved drugs. For aging: exercise, sleep, and not smoking. Melatonin itself, over the counter, has more human data than the peptide sold to restore it.
Latest
Developments since this edition, newest first. Items here are dated and sourced; they update the guide without re-editing it.
FDA warning letter hits a vendor that sells epitalon
Royal Peptides, which lists epitalon vials, received an August 24 warning letter for marketing seven other peptides as unapproved new drugs under research-only labels. Epitalon was not among the named products, but the letter's reasoning applies to any vial sold with dosing guidance.
Vote tally confirmed at 7 to 4; no compounding authority yet
A regulatory note lists epitalon at 7 yes, 4 no, and 1 abstention and says the vote did not change its status; adding it to the 503A list requires rulemaking that has not begun.
HHS secretary must still sign off before any pharmacy can compound
The pharmacists' association reports that the committee's six recommendations, epitalon among them, go to the HHS secretary for formal approval and that compounding cannot begin until rulemaking is complete.
Committee backs epitalon for insomnia over FDA objections
FDA reviewers told the committee they found no efficacy publication for insomnia and raised immunogenicity and a theoretical cancer risk from telomerase activation; the committee recommended listing anyway.
Independent UK study repeats the telomerase result in cell lines, cancer cells included
A Brunel University London group reported dose-dependent telomere lengthening in normal human cells through telomerase upregulation, and also in breast cancer cell lines through a separate lengthening pathway. It is the first replication outside the Khavinson institute, in a dish, and it sharpens the cancer question rather than settling it.
